Yes, see:

http://mvpmc.wikispaces.com/Mediamvp#toc3

It breaks down to two categories of machines referred to as non-flash
and flash. You D is the former and your H is the latter. They use
different bootloaders and thus require different things on the back-end
server to support the boot process.

The guide you followed did touch on the need to setuop a DHCP server to
support older hardware:
https://help.ubuntu.com/community/MythTV/MediaMVP_Frontend#Installation%20and%20Configuration%20of%20a%20DHCP%20Server

but it didn't go into the specifics.

There are a number of them linked from this page:

Try the ones marked "non-flash" for your rev. D hardware.

Possibly this old one from 2005 is the best for rev. D hardware:
http://mvpmc.sourceforge.net/mvpmc-HOWTO-singlehtml.html

You just need to use some judgment when following it, and skip the
obsolete bits, like where to obtain the mvpmc dongle. The wiki also has
better coverage of how to create a startup script ("config file") and
what to put in it. Generally the wiki has more up to date information on
specific areas, like DHCP server setup, so I'd do searches there as you
work though the guide.

(The howtos are rather a mess and need to be reorganized, but it's hard
to make that happen, as experienced users have moved on from setup, and
newbies don't know enough yet to know what needs to be reorganized. I
was hoping to someday create a "wizard" that would pose questions and
guide you to the best howto for your hardware and back-end combination.)

I believe it is possible to configure a boot server that will support
both hardware revisions, and I think a setup that works for rev. H,
should also work for rev. D.
