I believe I had the same issue. For me, I had the backend commflag
again and the issue was resolved.
I see in the log file that something is not correct. It appears that the
recordedseek table is bad.

(cmyth)cmyth_mysql_get_commbreak_list: COMMBREAK rows= 0
Found 5 commercial breaks for current program.
The first line says that the big query for the actual offsets in the
recordedseek table found zero rows... But the second line says that we looked
in the recordedmarkup table and found 5 commercials.
To me this says that the recordedseek table is bad/corrupt/something and that
the mvpmc can not find the actually offset into the recorded file.
Thats why I believe if you reschedule commflag for all recordings, it should
fix it.
The reason for the soft reset is there is no check between functions. We say
there are 5 commercials, but when we try to read the commercials offsets the
mcpmc resets.
