Could you check out https://gitorious.org/mypaint/mypaint/commit/f3d52866 and
see if that fixes it with a fresh config please?
For the missing icons, try rebuilding your icon cache as documented in README.
If it's still happening, please report it as a separate bug complete with
details of which ones are missing :)
