I have an MySQL instance running that is generating the =
innodb.status.#### files.  However, there is no innodb_monitor table in =
any of the databases.  How is this happening?

4.0.20 writes them automatically. Later MySQL versions only when you request it to do that.


Those files can be useful in debugging.
