You could try some variation of:
INSERT INTO inserttable (user, maildir)
SELECT REPLACE(user, '@', 'spam@') as user, CONCAT(maildir,'.spam/')
as maildir
FROM selecttable
[WHERE ..]
the where bit is optional of course!
