That is exactly what happened.  I have already apologized and sent off an
unsubscribe message with the correct email address.

It's something that may happen if you don't trim your quoted messages
when you're replying.  In one of your recent your messages you left
the whole message you were replying to hanging on the end.  That
includes the "unsubscribe" address that's personalized for you.
Someone clicked on it, probably unintentionally when trying to
unsubscribe herself.

The moral of the story is that you should never quote extraneous text
from the message you're replying to -- especially signatures and
the like.
