ArgUML (http://argouml.tigris.org/) is an open-source UML modeller written in Java, but you will have to code the DDL generator yourself. The commericial spinoff of ArgoUML is Poseidon, but the Poseidon folks told me last week that they have no plans to write a DDL generator.
