Extra shows if column has attribute AUTO_INCREMENT. Comments shows comment on the 
column level. For example:

mysql> show full fields from test;
+-------+---------+-----------+------+-----+---------+----------------+---------------------------------+-------------------+
| Field | Type    | Collation | Null | Key | Default | Extra          | Privileges     
                 | Comment           |
+-------+---------+-----------+------+-----+---------+----------------+---------------------------------+-------------------+
| id    | int(11) | NULL      |      | PRI | NULL    | auto_increment | 
select,insert,update,references | this is a comment |
+-------+---------+-----------+------+-----+---------+----------------+---------------------------------+-------------------+
1 row in set (0.01 sec)
