If you want to build something like a web form you will need to use HTML and a scripting language. A common solution to do what you are asking is to use PHP. You will need to be familiar with PHP to go further. Go to http://www.php.net to get a hold of the basics.
There is no way to write a walkthrough for your problem here as it would be quite long :o). It would also be repititive since there are millions of sites that provide such walkthroughs. Go to www.sitepoint.com, which is a web dev portal. My favortite.
