i had a problem where:
running apple os 10.3 server with mysql 4.0.16 and mysql would not start.
found this solution at: http://docs.info.apple.com/article.html?artnum=107537
problem was that mysql was unable to start due to the hostname on my system being set to something other than the DNS name. i.e. 'media' instead of 'media.arts.unsw.edu.au'. The article goes on to explain that this 'prevents the database from being created'.
It has the fix.
