You will need to construct a table in MySQL to take the results. You will need to create the relevant PHP script to receive the form data and build a SQL query to submit to the database. If you are entirely new to this then I would suggest taking a look at some examples on the web. Try: http://www.phpbuilder.com Another option would be to use Dreamweaver and have it write the code for you.
