I've tried the method suggested ( to add
Option "XaaNoOffscreenPixmaps"
in the xorg.conf). Now i have the tv running on the tv-out and on the monitor BUT IT'S EXTREMLY SLOW!! not only the tv, but also anything in KDE/Gnome!!
How to resolve that?

I don't believe you can have video overlay work on both displays. This is why you see a blue screen on one display and video on the other. If you use the shared memory driver (xshm in xine) instead of xv, you should see video on both displays at a cost of increased cpu usage.
