I'm no expert but I believe this might be caused by incorrect tuner
type in modprobe.conf.  My dual pvr-250s were not correctly identified
and so I had to manually set tuner type in my config.  Here is what my
modprobel line looks like:

# ivtv modules setup
alias char-major-81 videodev
alias char-major-81-0 ivtv
alias char-major-81-1 ivtv
options ivtv tuner=39,39

Again, not at all sure this is related but hey...worth a shot!
