well, you could just use gnokii or something like that and have a mobile phone connected directly to the nagios server, although I haven't tried this myself.

it does mean that there is a completely separate route for gettng alerts.

we send pages (no sms) using a modem on the nagios box and then this 
routes to the pager supplier - Relying on email or email-to-sms gateway 
is dangerous as what happens if all connectivity fails - your alerts do 
not get through.
