You need to have a machine that listens for SNMP traps. The program snmptrapd does this, it comes with net-snmp package. This daemon writes the trap info to the system log, or alternately runs a program and passes it the trap information. That program can then submit a check to nagios using NSCA or by writing to the nagios.cmd pipe.
