There is really no point, if it won't protect you in court anyway then I 
wouldn't even plant an idea in their mind that something could go wrong. 
Just make sure you have insurance and don't cross the line of what you are 
suppose to do. Something could happen, like an infection that is at no fault of 
yours but how can you prove it. You just need to make sure you follow the right 
sanitation procedures and safety precautions and have insurance incase 
something happens.

No. I have client cards that I use for record keeping. They let me track things 
like "Lisa H always seems to break the same nail" or gee, a lot of my clients 
are having problems with the after wax cream, things like that.

The problem with waivers is that they imply that something will go wrong, that 
you know it will wrong, and that you're doing it anyway. I know they seem like 
a good idea but they just escalate that whole climate of fear that needs to be 
eliminated with proper practices and a tech with a good education and work 
ethic. Keeping records of your client appointments, procedures, and outcomes 
will serve you better in the long run.
