What's a defect? That's a serious question, because you can always raise the bar higher and higher, especially if you want to talk about user interfaces and the ability to seamlessly meld with business process. You can also talk about how well the software matches one person or another's idea of good design. One article I usually reference is Martin Fowler's: http://martinfowler.com/bliki/VeryLowDefectProject.html There's another story floating around about a Navy training simulator that had zero field reported defects for ten years. And then there's my mother: all the programs she ever wrote ran correctly the first time. Both of them.
