The fan speed at the bottom of envstat output had me lookup how to set
it but I don't think hw.dbcool0.fan_ctl_0 is available on my ThinkPad.

Setting the cpu frequency to lower values by

   sysctl -w machdep.cpu.frequency.target=400

and running estd in in rc.conf

   estd=YES
   estd_flags="-d -l 40 -h 70 -b -M 2700"

lowers the temperature to 45degC in `envstat'

   [acpitz0]
          temperature:    50.000  127.000                             degC
   [coretemp0]
     cpu0 temperature:    47.000                                      degC
   [coretemp1]
     cpu1 temperature:    45.000                                      degC
   [thinkpad0]
        temperature 0:    50.000                                      degC

but the btop reading says 57degC.

Yes. The air vents at the bottom of the laptop look clogged.

Repasting the heat sink is beyond my competence.

Thanks for the help.
