This is what I did:
I copied the 0-newnat7.patch to /usr/src/linux.
Then I applied the patch manually with patch -i 0-newnat7.patch -p1
After that I returned to the iptables directory and runned:
./runme newnat
This worked fine for me, except for the the bugfix in the h323 module I
discovered.
I posted the bugfix yesterday on this list.
