How apply patch-o-matic ?????

Hi, i�m going mad because i cant apply patch-o-matic.
I have kernel 2.4.18 just untared,latest iptables....
Any one could be so kindly and tell me the correct way to apply this patch 
so i can use h323, or newnat patch...
what version of cvs needed????
Normaly first i do make menuconfig and make dep so dependencies are 
created, then make patch-o-matic, and nothing,
newnat apis dont apply correctly, or with runme newnat.....
