You shouldn't have to use netfilter for this as ssh can do it all by
itself, look at the -L option.  If I remember correctly:
  ssh -L 12000:clown:5907 my.nat.box.com
will let you set your local VNC session to connect to localhost:12000
and ssh will forward it along to the box that you're ssh'ed into and
then over to the clown computer.
