Not sure what is going on here, but since the nsurl fetcher changes I noticed NetSurf was not fetching pages quite as fast - appearing to download everything before displaying the page. Having just downloaded a large file, I can see that the download process is blocking all browsing, so I have come to the conclusion that the recent changes have caused a fetch to block all other fetches.
