The driver is unable to automatically determine your vidmem, and is defaulting
to 1MB, which causes almost all your modes to fail because of insufficient
memory. Add the following line:
VideoRam 8192
to your Device section (you might need to change the value if you don't
actually have 8MB of vidmem).
