Not sure exactly what causes this, some rambling thoughts that may help: Despite the all the awesomeness of urpmi, this does seem to happen far more often than I would like (even once is far too much). Usually I chalk it up to network problems and just resolve to trying again later. You say this happens with the CDs too. Are you sure? Sometimes it has helped me to remove all my sources except the CDs and try it. You can always go back to "easy urpmi" and re-add your network sources later. Usually I have removed the CD sources altogether because that has seemed to help me--just have one main, one update, and one contrib (and one plf if you like) source. Have you done a urpmi.update -a lately? Did you see any error messages when you did that? You may want to try using a --clean or something like when you do the urpmi mozilla, I'm not at my Mandrake box right now so you'll have to check the man page yourself on what the exact option is (if it even exists...it's been a while).
