Ben, if you're using Maildir, I'd be surprised if that was not a bug in the imap oder smtp implementation you are using.
I've been (with others) administering a very similar setup on two "larger" installations (whatever that is) using exim and courier and have never noticed this to become a problem - but those systems were all s9 and s10. The fact that, to get a .nfs file, the unlink must have been done on the same server which had the file open (otherwise you simply get stale file handles on the server holding the file open) might help finding to the root cause.
