Given that it happens only sometimes, I'm guessing maybe "operator
error" caused by a lazy foot.

Before you tear things apart, convince yourself that it actually is an
issue.  Your bike may just be training you on how it wants to be
ridden.

Every bike I've ever owned (and friends' bikes ridden for the first
time) required a "period of adjustment" to the bike's particular
personality.  Some shift sloppily, some tight, some easy, some
stubborn...and like that.
