I thought I'd dig into this a bit.
v3: foo.md -> foo/index.html
v4: foo.md -> foo
To make foo.md to turn into foo.html, in v4 it needs to be called foo.md.html
That's pretty awful and I'm debating just dumping middleman entirely as a
result.
