The issue is with the order in your actions

  actions = [[openflow.OFPAT_OUTPUT, [0, output]],
[openflow.OFPAT_SET_VLAN_VID, 2]]

Change it to this:

  actions = [ [openflow.OFPAT_SET_VLAN_VID, 2]
,[openflow.OFPAT_OUTPUT, [0, output]] ]

and then it should work I think.

Right now your flowmod first outputs the packet and then adds the tag.
You need to switch the order.
