No online defragmentation is planned (offline defrag should help), that 
wasn't supposed to be the solution. The metadata of the file should be 
extended in a rare, error-prone, risky way. But as a first step it would 
be just much better to avoid getting into such scenarios for several very 
good reasons (reliability, interoperibility, performance).

Estimate can't be really given since people are working on the driver 
voluntarily when they have time, moreover feedbacks largely influence 
priorities. 

The current roadmap, in top-down priority

  1. reliability (investigate, reply plenty emails, fix, document minor
     usability issues and major misconceptions; this takes most time)
  2. improve write performance of multi GB large files on close to full,
     fragmented volumes 
  3. handling a rare delete scenario
  4. perhaps the above file creation but 2. will also improve on it as a
     side-effect, and it will make such scenarios disappear most often
