First question: \setuplayout[marking=no] or is it marking=off? One of the two. Second question: I have no idea. One possible thing which has arisen for me many times: assuming you are printing from Acrobat, are your print settings set to either enlarge or shrink pages? This is something I know I have gotten wrong more than once.
