Can one redefine the TeX output macro as one can within TeX?

You mean installing another output routine?
In principle one can define his/her own otr (with fallback on an existing one); see page-one.tex, page-mul.tex, page-set.tex for details.
It also depends on what you want to do. There are quite some hooks into the otr already, so what do you hav ein mind?
