Hmm. Try this:

% look at enco-def for guidance
\buildtextaccent\textdotaccent n

\definecharacter ndotaccent       {\buildtextaccent\textdotaccent n}
   \ndotaccent

%enco-acc provides some shortcuts:
\defineaccent . n {\ndotaccent}
   \.n

\defineaccent " n {\ndotaccent}
   \"n

...as you're making a composite glyph, YMMV with different fonts, as you
saw with variable macron spacing. If you want to activate the ", then
you're on your own...
