Thanks for your reply. With the following example I still have 4 pages, 

the front 
[empty]
the back
[empty]

%%% start example

\setuppagenumbering[alternative=doublesided]
\setupmakeup[page=no,doublesided=no]

\starttext

\startstandardmakeup
...the front
\stopstandardmakeup
\startstandardmakeup
...the back
\stopstandardmakeup

\stoptext
%%% stop example.

However, defining a custom makeup works.

%%% start working example
\setuppagenumbering[alternative=doublesided]
\definemakeup[mypage][page=no,doublesided=no]

\starttext

\startmypagemakeup
...the front
\stopmypagemakeup
\startmypagemakeup
...the back
\stopmypagemakeup

\stoptext
%%% stop
