TM is high already, but with (R) you'll need that. This is what I
ended up with years ago, before I went UTF-8 (that simplifies things
quite a bit):


(R), up high:
\high{\registered}

TM, default placement:
{\trademark}

TM, extra high up:
\high{\trademark}


I can't recall where I managed to find these in the end, but I
remember that I really badly needed (R) for this:
Teflon\high{\registered}

:-)
