Just speculating: is it possible that the glyph is being searched for
in Iwona-Regular.otf instead of taking the glyphs from tfm/Type1 math
font?

in the virtual font we start out with a regular otf text font and overload slots using the type ones so the fact that one gets a Iwona-Regular reference means that 2016 is not overloaded
