I’m afraid that the issue with "focus=width" is a bug in evince or poppler.

I’m afraid that link destination is miscalculated by ConTeXt. But in the
previous sample, evince should behave the same either with "focus=width"
or "focus=standard".

Links have internally the same top value:

    << /D [25 0 R /XYZ 70.867 689.409 null]>>

    << /D [27 0 R /FitH 70.867]>>

I will ask to the evince developers about it.

mupdf-1.6 doesn’t have this issue (and it’s way faster than evince).
