Thanks Wolfgang.  The problem I had with using headers was maintaining the
numbers for the headings and clauses.  I was able to solve this with the
[prefix=yes] option for the itemizegroup.

Here is a solution (for anyone else) that achieves what I want:

\chapter{A heading}

\startitemize[n,repeat,2*broad][prefix=yes]
    \starthead{A sub-heading}\stophead
    Optional intro text here:
    \startitemize[n,repeat,3*broad][prefix=yes]
        \item \input knuth
        \item \input tufte
    \stopitemize
    \starthead{Another sub-heading}\stophead
    \startitemize[n,repeat,3*broad][prefix=yes]
        \item A short clause
    \stopitemize
\stopitemize

\chapter {A new heading}

\startitemize[n,repeat,2*broad][prefix=yes]
    \starthead{A sub-heading}\stophead
    \startitemize[n,repeat,3*broad][prefix=yes]
        \item \input knuth
    \stopitemize
\stopitemize

\stoptext

I suppose I could make the sub-heading a \section too.  I just need to check
my document that all level two items are heading.

The best solution is to use real headers and drop the enumerations.

You can hide disable parts (e.g. the chapter number) of the section counter
to get the same results as you have with your current solution.
