Use notepad

Start the file with regedit 4
Put the add/ modify key and command in afterwards. 
Save as file.reg

For example:

<SNIP..>
REGEDIT4

[HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows\CurrentVersion\RunServicesOnc
e]
"W95Y2K"="c:\\windows\\temp\\w95y2k.exe /q"
<SNIP..>
