Second: on what format should the image be installed, hfs or linux?
Third: let's assume the worst, that a CD player is NOT available for an
install. Will the floppy image allow for a net install going to/from a
B & W to install to the 6100? Or is a net install impossible for nubus
PPC? Will the floppy-based install find the install files on a 6100
hard drive (and are these files on a linux or hfs partition)? Thank you.

latest miboot image from CD. Please see our files section for the
miboot images that come with the Debian Installer for Sarge.
However I don't know whether it is possible to directly boot from an
external SCSI CD.
You could alternatively use a floppy image.
