I've seen these when a render is aborted before Nuke starts calculating it (in my case it was a programmatic render cancelation from a 'beforeFrameRender' callback). As of 6.3, the EXR writer (and possibly all file writers) is much more aggressive about initializing output file handles, so what happens is the file is opened, Nuke writes the header and possibly a small amount of data, the render is canceled, and then the file is closed properly. I can't say I've seen it happen as a result of trying to render standard nodes, though... Perhaps there's an error causing the render to abort (I'm guessing you've checked the terminal already), or maybe it's one of the bogus licensing errors Keylight, Furnace Core et al. are prone to in random situations.
