Unless you set values when outputting the form, it's most likely the browser doing it. I suggest trying another browser or temporarily disabling the remember passwords feature.
As for the PHPSESSID cookie, that's the cookie PHP uses to store the user's session ID so sessions aren't lost when moving pages.
