Implemented LoadTilemap to replace old xbload for the .T lump
* Lots of related cleanup
* map buffer no longer dynamically sized, now it is
  always big enough for max size 3-layer map
* Fixed a nasty bug in game's layer implementation, causing
  out-of-range memory to be displayed as upper layer data
  when the map has an area larger than approx 103x103
* Bug 308 not fixed yet. definitely not a load problem.
