Here's how to reproduce the bug.
First open up two terminals.
Execute the command
okular --unique 'document.dvi#src:100 ./document.tex'
This opens up the file normally, but now execute the same command in the other
terminal.
Then Okular produces an error
Could not open file:///home/user/document.dvi%23src:100 ./document.tex
