This error is mostly caused by the editor misbehaving; you can expect it more frequently when you edit pages. (Please do not yet edit CDs, you will break them! Yes, it's no surprise that you were logged in and you user account was created. SWiM (actually the underlying IkeWiki system) is heavily based on JavaScript, so this misbehaviour occurs on the client side only. The server side is not affected by that and works fine. (And the code on the server is much easier to debug than the client-side code -- that's why there are many actual bugs on the client, while it's rather missing features on the server side.)
