I'd suggest the email form, as Louis suggests, especially if said email has to go to ooo-private, as Rob said. ("Rule one is, we don't talk about ooo-private...") It can be at the end of, or linked to from, a FAQ. Simple and quick for the user, and can include a subject tag for easy identification by the list moderators.
Also transparent to users if you later decide to send said requests to, say, ooo-marketing. Don
