I found the problem with this.  This error:

! pdfTeX error (ext4): \pdfendlink ended up in different nesting level
than \pdfstartlink.

appears to be a known bug in TeX or pdfeTeX.  (I'm not sure quite what
is what.)   You can find references using Google.  Here's one:

It has something to do with a hyperlink breaking across a page
followed by a longtable.  There doesn't seem to be any fix for it
except for moving things around so the offending hyperlink doesn't
break across a page.

In one chapter I had two big sections where I wanted to switch the
order of the two. When I move the one section above the other I get
the error.  Moving the section back, eliminates the error.

That's good enough for me now. But, it seems like this could come up
at any time.  So it is something for us all to be aware of.
