is it correct that a No-Op is handled entirely withing the iSCSI layer (i.e. no 
command is sent to the actual device), while a TUR (multipathd) is answered by 
the 
device?

I saw that the Microsoft Initiator is using No-Ops. Using TUR with Linux, I had 
some cases where the reply timed out. So that could be a network problem or a 
device problem. If the No-Op is handled without the device, I could change the 
test to find out what part is causing the delays.
