But, I'll make a guess. If this patch finds it, I'll be surprised. If it doesn't, I'll guess there's an uninitialized mutex, but I'd have to look harder to see if that's detectable.
I can report that the patch does not help klog crashing an AIX 5.1-06 system... In nosing around the crash dump (I'm also unfamiliar with kernel debugging and AFS kernel internals), it does appear that it might be related. Here's a quick postmortem status:
