The best option is to place a device in front of the server to catch this stuff (a separate machine running the daemons in the threads below), because by the time your web server gets all this - - it's basically too late.

I use CloudFlare.com, because they provide this as a service.

I've also tried CFTHROTTLE, with less than consistent results. It was a nice idea, but is probably deprecated now.
