This is wrong (I think) in all template files.
This is a mistake introduced with configure_etc.sh. I added a comment to config.xml which exlains the usage.
Also, on the offline CA, should httpd_host be the server for the offline CA server, or the public RA server?
It is at every the public server.
I set it up to the the offline CA server, but it seems to encode it for the CRL URLs in the certificates that get signed.
Yes and this is the only usage of httpd_host and httpd_port.
As a very minor issue, configure for some reason detects my version of openssl as being version 0.9.87?

No, but where does configure test the version of OpenSSL? configure should only test the features of OpenSSL but not the version.
